Request Letters to Professors
As a client of the Learning Diagnostic Clinic (LDC), you are eligible to have a letter written to give to your professors; the letter states the accommodations you are to be provided as an "otherwise qualified" individual with a disability. It is the student's responsibility to get the letters to their professors ensuring that the student has contact with his/her professor about his/her needs. At this time, please read and fill out the next portion of this information sheet. By filling this information out, you are declaring that you wish to have a letter detailing your accommodations written for the professors that are listed on this sheet. Understand the letter will only inform the recipient that you are a client of the Learning Diagnostic Clinic and what your specific needs are.
